TA Hour Log Sheet
This form is intended to be used by members to keep track of their hours for personal use. It is not to be used as a time sheet to be submitted to the employer.

Application Form
This form, Appendix D in the Collective Agreement, is used to apply for TA positions. The PDF can be filled out on-screen and printed out. A copy of this form must be dropped off at each department you’re applying to, and a copy brought to the Union office (the 2nd floor of the UC, above The Daily Grind).

Job Posting
This form, Appendix C in the Collective Agreement, is used by departments to post positions for hiring.

Offer of Appointment
This form, Appendix E in the Collective Agreement, is used by departments to offer positions. Once you have received an Offer of Appointment, you have a job.

Assignment of Work Agreement
This form, Appendix F in the Collective Agreement, is completed with your supervisor, and outlines how your hours are to be broken down for the semester. If you find part-way through the semester that there are issues with how your hours are set, you have the right to meet with your supervisor to adjust them.

Notice of Application Form
This form, Appendix E in the Collective Agreement, is used to apply for Sessional positions when you already have an application on file with the department in question. You are strongly encouraged to update your CV on file with the department if anything has changed since you applied last.

This form, Appendix F in the Collective Agreement, is used by departments to offer positions. Once you have received an Offer of Appointment, you have a job.
